Job Description
Age UK South Cumbria has to vacancies available for the position of: Home from Hospital Support Officer.
-Supporting local hospital teams in providing timely and effective discharges for older people
-Transporting older people home from hospital
-Ensuring a safe and sustainable transition home
-Conduct a holistic assessment of their needs
-Provide short term assistance to promote their independence
The two positions are based out from our Kendal office or Barrow-in-Furness office.
35 hours per week
Mon-Fri 9am-4.30pm
Fixed 12-month contract
Salary: £24,406 - £26,007 pro rata
NB: No personal care will be provided by the post holder.
Full UK driving license essential.
